We just got this achievement and only got the normal loot from the tribute chest along with 4 Trophies. Most likely the cloak only drops with Tribute to Insanity chest

Yeah, this was a shocker. We got it with 48 attempts remaining and were all excited to get a 258 cloak... that wasn't there.

The loot received from 10 ToGC is only ilvl 245 atlas loot isn't wrong, even from the chest the loot is 245, Loot with ilvl 258 is obtained from the tribute chest in 25 ToGC.

After a guild run/pug did 5 attempts, my guild decided to pick it up tomorrow. With 45 attempts left, we 1 shot all the bosses and got this achievement. Moral of the story is to go in with 50 attempts :(
